RSS Forms

The following forms need to be completed by those interested in participating on applying as a participant on the Rural Social Scheme in the Cavan-Monaghan region.

• Participant Application Form 2009-2010
• Electronic Funds Transfer Form

If the applicant is currently using someone else’s Herd number then this form should to be completed:

• Use of Herd Number Form

If the applicant has any children over the age of 18 and under the age of 22 in Full-time education this form should be completed:

• Child Dependents in Full time Education

The following form is for CURRENT participants of the Rural Social Scheme who have a change of circumstances: e.g. birth of a child

• Form RSS4

The following form is for community groups or other local bodies who wish to apply for RSS workers assistance and become a RSS Sponsor Group

• Group Application Form

All forms should be returned to Cavan-Monaghan LEADER marked for the attention of Rural Social Scheme. Further assistance is available by emailing the RSS Coordinator or by contacting the CML offices on 049-4338477.
